Physical Exam Requirements
Students eight years old and younger
Every student age 8 or under entering a Kansas public or private school for the first time must turn in a physical exam form to the school. If a physical exam has not been completed at the time of enrollment, a “Physical Exam Exemption Form” must be signed and turned in to the school. The Exemption Form gives the student 90 days from the date of the enrollment to have the exam completed and the form returned to school.
A physical exam completed 1 year prior to the enrollment in a Kansas school may be used to fulfill this requirement. Please check with the school nurse to see if a previous exam fulfills the Kansas requirement.
A physical exam form may be downloaded from Health Forms- Elementary School Forms. Forms from other schools/states are accepted.
Students in grades 6-12 wishing to participate in Kansas State High School Activities Association
sponsored activities/athletic programs
Physical exams Required for participation in Middle School and High School Athletic/Activities sponsored by the Kansas High School Activities Association.
Students must pass a physical exam with documentation on the Kansas State High School Activities Pre-Participation Physical Evaluation form. The physical exam must be completed after May 1st of the academic year for which participation is desired.
